Requirements for minor piercing with parental consent

1. Must be at least 16 years of age.
2. Must be accompanied by a legal guardian, both minor and guardian need state or federal photo id's with matching last name and address. Id's that work are passports, driver's license, and state identification card. School id's, birth certificate, social security cards do not work. If last name or address is different, court papers stating guardianship will work.
3. No nipple or genital piercing will be performed on any minor.
